You can get such forms at http://findlegalforms.com. They usually cost around $5 via paypal. This is a pretty good deal for a legal form that is written by a lawyer.

Though it should be noted, that nothing will cover trademarking. Trademark and copyright are different. Whoever creates the work automatically owns the copyright, and the forms available at that site will allow you to have the copyright and ownership of the work passed to you. In order to have something trademarked, you have to either use it or register it. If you want to trademark a business name or logo, it is suggested that you register it with the government. In the US: www.uspto.gov
